Job Type
On site, FTE: 1.0, Standard Hours: 40.0, Shift: Shift 1 (8:00am-4:30pm). No weekends or holidays.
Job Summary
Responsible for coordinating the discharge plan of care for all patients during their hospital admission. Coordinates the discharge plan starting on admission and through discharge follow-up. Provides biopsychosocial assessments and updates that address the patient’s evolving discharge care needs to timely move them to the appropriate next level of care. Establishes a plan of care that provides maximum benefit to patients and their families in collaboration with the multidisciplinary team in Care Coordination Rounds (CCRs) seven days a week.
Experience
A minimum of two years experience in an acute care setting is preferred. Casework in medical social work is preferred.
Education
Master's degree in Social Work is required.
Special Skills
Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients and families. Analytical skills necessary to assess patients’ needs, develop associated discharge planning and develop solutions to problems concerning patients’ mental and physical well being.
License
Current State of Wisconsin license as a social worker is required.
