Job Summary
The Nurse Coordinator plays a vital role in the Lung Transplant Program(s), offering direct care to patients with complex medical conditions. They serve as the primary point of contact, coordinating pre‑transplant evaluations, managing post‑transplant follow‑up care, and providing patient education. This position requires collaboration with a diverse team, including physicians and support staff, to deliver exceptional care and manage the unique needs of transplant patients.
Responsibilities
Schedule patient evaluations, education, and orientation to the transplant process.
Monitor the evaluation process, ensuring timely progression of patients.
Liaise with multiple hospital departments during the patient evaluation process.
Facilitate patient activities with consulting departments and physicians.
Communicate effectively with peers, other healthcare team members, and external contacts.
Assess patient and family needs and communicate these needs to appropriate services (financial counseling, social work, nutrition, etc.).
Provide both inpatient and outpatient care for patients throughout all phases of the transplant process.
Coordinate all post‑transplant patient care per physician orders and protocols in the outpatient and clinic setting.
Attend and participate in weekly heart or lung transplant patient selection meetings, documenting decisions and following up on actions.
Manage logistics of donor offers for heart and/or lung transplant patients during regular working hours and cover after‑hours, holidays, and weekends as needed.
Coordinate all teaching for pre‑ and post‑transplant patients and provide cross‑coverage to other coordinators for vacations and sick leave.
Assist with the maintenance and development of the heart and lung database(s).
Stay abreast of changes in federal, state, or local regulatory requirements related to transplant services and reporting.
Maintain communications with UNOS regarding patient listing, status changes, letters of appeal, and requests for pre‑transplant data and removal from the waitlist.
Assist financial counselors in obtaining insurance authorizations by providing patient information to insurance companies.
Participate in all supported research protocols as required.
Qualifications
Minimum of five (5) years of adult solid organ transplant nursing experience as an outpatient or inpatient transplant coordinator.
Five (5) years of RN experience in end‑stage organ‑specific disease, transplantation, and managing patients with organ failure.
Proven organizational skills and the ability to work independently, managing multiple concurrent tasks in a high‑stress environment.
Excellent communication skills, both written and verbal, and a professional demeanor.
Computer literacy, including proficiency in MS Word, Outlook, internet use, and a willingness to learn and adapt to new EHR systems.
Strong MS Office skills, particularly in Word, Excel, and PowerPoint, and proficiency in using databases.
Active Registered Nurse (RN) licensure in the State of California.
Active American Heart Association BLS certification.
Preferred: BSN or MSN degree, and experience in assessing advanced heart failure/cardiac and/or pulmonary disease.
Clinical competency in managing transplant candidates and recipients, with EPIC/APEX experience highly advantageous.
License / Certifications
Active Registered Nurse licensure in the State of California.
Active American Heart Association CPR certification.
