Overview

The RN Clinical Nurse in hospice is a proactive member of an interdisciplinary team of licensed and unlicensed caregivers who ensure that patients, families, and significant others receive individualized high-quality, safe patient care at end of life. The nurse provides hands-on patient care at the bedside in Hospice and Wellstar hospital settings, under the supervision of Nurse Managers and other leaders. The practice follows the WellStar Professional Practice Model, 5-Star Nursing, and evidence-based practice. RN competencies are derived from these models and supported by the WellStar Core Competencies. The nurse upholds the voice of the patient and adheres to department, system policies and procedures while supporting service excellence goals.

Core Responsibilities and Essential Functions

Delivers direct nursing services to terminally ill patients and provides supportive services to their families with compassion and sensitivity to physical, social, cultural, and spiritual needs.
Performs the Nursing Process (assessment; diagnosis; identification of outcomes; planning; implementation and evaluation).
Provides relationship-based, patient-centered care aligned with population-specific characteristics and cultural diversity.
Partners with the patient and significant others to keep goals central to the plan of care.
Documents all care in the EMR accurately and timely.
Explains palliative approach to care and assesses goals for care, including DNR preferences and hospice benefits.
Obtains consents, election statements, and determines the patient’s attending physician; develops initial Plan of Care.
Provides ongoing education to patient, caregivers, and family regarding disease progression and anticipatory grief.
Provides emotional and spiritual support; promotes a safe and peaceful environment; supports end-of-life goals and advance directives.
Facilitates medication management and administration; assesses symptoms and responds to promote maximum comfort; maintains medication records and follows policy for storage and waste.
Coordinates safe, quality patient care through interdisciplinary communication; supports timely documentation and inclusive clinical environment.
Develops Personal Care Plan and supervises CNAs; demonstrates fiscally responsible teamwork and supports recruitment and retention of staff.
Participates in performance improvement and collaborates with stakeholders to promote safe, quality patient care.
Completes required professional competency assessments and mandatory education; serves as preceptor/mentor to ensure a qualified workforce and healthy work environment.
Adheres to all system policies and regulatory standards, including patient safety goals and safety absolutes; engages in data collection and evidence-based practice as appropriate.
Facilitates transfers and discharges from hospice and coordinates continued care with providers.
Contributes to development of protocols and procedures in collaboration with care managers and health care team to achieve best practice outcomes.
Required Minimum Education

Associates Nursing or Diploma in Nursing; or Bachelors Nursing (preferred).
Required Minimum Licenses and Certifications

BLS – Basic Life Support (or ARC-BLS, BLS-Instructor).
RN license (single state) or RN-Compact (multistate).
Georgia Driver’s License (or valid state driver’s license).
Private Vehicle insurance (PVI).
Required Minimum Experience

Minimum 2 years of acute care experience as an RN.
Required Minimum Skills

Ability to read, write, and speak English; proficient with technology to support clinical care; basic computer skills; strong interpersonal and collaborative abilities.
