Assist in daily living activities, ensuring patient comfort and safety.
Document patient care and progress accurately in electronic health records.
Collaborate with registered nurses and physicians to develop and implement patient care plans.
Educate patients and families on health management and disease prevention.
Maintain a clean and organized environment in accordance with healthcare regulations.
Participate in staff meetings and training sessions to enhance professional development.
Qualifications
Valid LPN license in the state of Maryland.
Minimum of 1 year of experience in a healthcare setting, preferably in home health or long-term care.
Strong communication skills and a compassionate demeanor.
Ability to work independently and as part of a collaborative team.
Basic Life Support (BLS) certification required.
Proficient in electronic health record (EHR) systems.
Commitment to providing high-quality patient care and support.
Flexibility to work various shifts, including evenings and weekends.
