Non-Clinical Case Manager

The Lead Non-Clinical Case Manager provides comprehensive non-clinical case management and care coordination services in support of the Vermont Army National Guard (VTARNG). The position supports service member wellness, operational readiness, and deployability by coordinating resources and services across VTARNG facilities and external organizations.

Duty Location
Green Mountain Armory, Colchester, VT

Work Schedule

Monday through Friday

Typical duty hours between 0730 and 1630

Support for weekend Soldier Readiness Events (SRP/PHA) and mobilizations as required

Primary Duties and Responsibilities

Coordinate all aspects of the care of individual Service Members

Ensure proper utilization of services and resources

Establish and maintain an eCase profile for Soldiers referred by a provider

Track Soldiers' care from identification through resolution

Maintain HIPAA standards and guidelines

Document care in eCase, MEDCHART, MEDPROS, eProfile, MHA/PHA, AHLTA, AVS, MODS, and MWDE

Prepare Soldier records for Medical Evaluation Boards (MEB/PEB)

Coordinate treatment plans with appropriate medical facilities

Maintain Soldier medical records in accordance with AR 40-66

Produce monthly case status and profile reports

Upload medical documents to Health Readiness Record (HRR)

Additional Responsibilities

Attend meetings as needed

Maintain required records and documentation

Support administrative and coordination tasks

Process Improvement and Continuity of Operations
Assist with process improvement and continuity of operations

Minimum Qualifications

Registered Nurse (RN) with an active, unrestricted active Vermont state license that is in good standing

Experience in non-clinical case management or care coordination

Ability to coordinate services across multiple organizations

Strong organizational, communication, and interpersonal skills

Ability to work effectively with military service members and Government personnel

Proficiency in written and spoken English

Ability to obtain a Common Access Card (CAC)

Maintain HIPAA and PII compliance at all times
