Job Details
Salary: $105,000 - $117,800 per year.

Responsibilities
Manage a caseload of up to four terminally ill patients per day.
Generate and follow individual plans of care to provide maximum comfort and quality of life in home, skilled nursing facilities, and inpatient unit settings.
Coordinate care efforts among LPNs, HHAs, and other caregivers.
Educate patients, families, and caregivers on medical choices and treatment options.
Assess patients weekly and as needed to identify the necessity for plan of care revisions.
Gather information on patients’ psychosocial and spiritual needs for symptom management.
Assess terminally ill patients in crisis for need of inpatient transfer for symptom management.
Maintain up‑to‑date computerized patient records.
Communicate with relevant physicians, staff members, and other agencies.
Assess patients for recertification eligibility, recertify patients, or plan safe discharge.
Schedule
Monday through Friday from 8:30 am to 5:00 pm. Includes one weekend per month.
