Position Details
The Social Worker intervenes with patients who have complex psychosocial needs, requiring assistance with eligibility determination for social programs and funding sources and qualifying for community assistance from a variety of special funds and agencies. In addition, the Social Worker offers crisis intervention to patients and families and coordinates the development of a discharge plan of care for high-risk patient populations. They also conduct behavioral health/lethality assessments, provide recommendations, and arrange placement in inpatient psychiatric hospitals as indicated. The worker must demonstrate knowledge and skills necessary to provide patient-appropriate care and understand principles of growth and development across life cycles.
Shift: The worker covers shifts at Mount Carmel Dublin and St. Ann's, working 8‑ or 12‑hour shifts, rotating across days.

Requirements / Qualifications
Master’s degree in social work from an accredited program.
Licensed Independent Social Worker in the State of Ohio (LISW).
At least two years of recent discharge planning experience in an acute care setting.
Knowledge of basic medical terminology and basic computer skills.
