Job Description Summary

MUSC Community Physicians (MCP) is an entity within MUSC that provides healthcare to patients throughout the state of South Carolina. Under the clinical practice guidance of the Social Work Supervisor, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
Key Responsibilities

Complete psychosocial assessments on patients and caregivers throughout hospitalization.
Arrange outpatient follow‑up as needed.
Provide direct clinical social worker services in an acute care psychiatric setting, including psychosocial assessment, active treatment (group therapy), discharge planning, and case management for a complex caseload.
Identify risk factors and apply discharge interventions to lower patient risk of undesirable events post‑discharge.
Bridge the transfer to outpatient providers with patients and caregivers to address issues and questions.
Work 6‑hour shifts on Saturdays and Sundays, with a scheduled weekly total of 12 hours.
Requirements

Master's degree in social work or a social welfare program accredited by the Council on Social Work Education.
One year post‑graduate professional social work experience; three years preferred.
Licensed as LMSW/LISW by the South Carolina Board of Social Work Examiners; LISW‑CP licensure preferred.
Excellent communication skills, attention to detail, and ability to work independently.
Ability to collaborate positively and professionally with medical professionals.
Ability to work in a thorough but efficient manner.
Certified in Safety Care and Basic Life Support within the first 90 days of employment and annually thereafter.
