Summary
The Social Worker manages the social, emotional and financial wellbeing of patients identified as needing the services of the Clinical Social Worker. They develop patient discharge plans, coordinate required services upon discharge, create initial and ongoing assessments, provide psycho‑social intervention, accept referrals from outpatient areas, refer to community agencies, and provide education and consultation to nursing staff. They complete psychiatric evaluations and placements for patients presenting to the ED with mental health or substance abuse complaints.
Responsibilities
Develop individualized plans with Physicians and Hospitalists for appropriate services.
Collaborate with RN Case Managers to manage clinically complex discharges.
Attend daily rounds with RN Case Managers to discuss assigned patients.
Coordinate discharge and follow‑up activities.
Discuss financial ramifications with families and provide guidance in placement issues.
Provide individualized psychotherapy, supportive counseling, and group therapy as needed.
Make referrals to community agencies when appropriate.
Identify potential legal or risk management issues and discuss with Director/Manager.
Ensure resources are managed cost‑effectively while achieving positive clinical outcomes.
Identify service needs, system issues, and opportunities for expansion of services.
Participate in continuing education programs relevant to the job function.
Act as a patient advocate.
Document assessments, proposed plans, changes, and final discharge plans in the medical record in a timely manner.
Communicate with Skilled Nursing Facilities, Community Health Agencies, Physicians, and other staff in a timely manner.
Complete statistical reports as requested by supervisory staff.
Educate primary care providers and clinical staff on alternative care options, including high‑tech home care, skilled nursing facility capabilities, and disease‑management initiatives.
Educate patients regarding managed care and Medicare processes and regulations.
Provide patient teaching to patients and/or families.
Promote Clinical Social Work services within legal, ethical, and professional standards.
Demonstrate effective leadership skills and serve as a resource for members of the Case Management department.
Attend Utilization Management Committee meetings as assigned.
Participate in facility quality‑improvement plans through unit or divisional activities.
Maintain current knowledge of regulatory requirements.
Collaborate with other agencies for education and benefit of staff and hospital.
Represent the department on hospital‑wide teams to improve service to patients.
Evaluate psychiatric patients in the Emergency Department and make recommendations to the ED physician regarding disposition.
Complete psychiatric/substance‑abuse evaluations on medical floors based on psychiatrist availability.
Complete assessment and disposition of patients presenting with abuse
eglect concerns and report to appropriate agencies.
Meet and maintain requirements for Medical Staff Office Credentialing as Independent Practitioner.
Minimum Qualifications
At least two years of case management and managed care experience.
At least two years of experience in a clinical setting.
Experience developing and facilitating patient discharges from an acute care setting.
Excellent assessment, diagnostic, and crisis intervention skills.
Master’s in Social Work.
Current Massachusetts license as LICSW.
Certification in Case Management (preferred).
Physical Requirements
The employee may perform or be exposed to infectious/contagious diseases (25‑50%). Vision requirements include close vision, color vision, and ability to adjust focus. Safety equipment used while performing this job includes eye glasses, face shields, surgical mask, surgical gown, and gloves.
Pay Range
$67,724.80 – $111,716.80
