Job Overview
Central Coast Home Health & Hospice is a fast‑growing, locally owned and operated organization with an excellent reputation throughout the Central Coast. We are now seeking a full‑time Medical Social Worker or Social Worker Assistant to work with both Home Health and Adult Hospice patients in the Southern San Luis Obispo areas (5 cities – Lompoc).
This role offers the opportunity to make a meaningful impact while enjoying autonomy and flexibility, supported by clinical supervision from an experienced LCSW. Clinical supervision is available for candidates working toward licensure.
Requirements
Bachelor's Degree
in Sociology, Psychology, Human Development OR
Master's Degree
from a school accredited by the Council on Social Work Education
Minimum of one‑year social work experience in a healthcare setting; home health and/or hospice experience preferred
Understand the hospice philosophy and issues of death and dying
Reliable vehicle to drive for work
Current driver’s license
Clean background and driving record
Maintain auto insurance coverage
CPR certified
Attributes
Act calmly and professionally at all times
Strong organization and communication skills
Work well independently and in a team environment
High attention to detail and good follow‑up
