RATE:

$24.00/hr. to $27.86/hr. (depending on experience)

JOB SUMMARY:

The social worker works directly with residents to identify their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the CMS and NYS regulations along with strict adherence to facility policies. The social worker is responsible for fostering a patient centered care, policies and routines that enable residents to maximize their individuality, independence and dignity. This patient centered care provides residents with the highest practical level of physical, mental and psychosocial well‑being and quality of life.

Responsibilities:

Upon admission the social worker will complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.

Upon admission the social worker will determine the resident’s code status, maintain advance directive paperwork/policies at all times ensuring 100% of facility‑wide accuracy, and assist residents/representatives in modifying advanced directives when desired.

Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues, the goals to be accomplished, and the appropriate social worker interventions.

Facilitate Care Plan meetings with resident/family and the interdisciplinary team at admission, quarterly, annually and for significant changes.

Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long‑term care facility, including their social, emotional, and psychological needs.

Ensure or provide support and education to residents/family members/significant others to assist in understanding placement and facility issues, and refer them to appropriate services when needed.

Act as staff liaison for resident counsel and family counsel groups.

Provide clinical interventions to address catastrophic events that occur during the resident’s stay.

Act as discharge planner by identifying residents’ discharge plans and coordinating referrals for appropriate home‑care services prior to the resident’s return to the community.

Meet with and provide counsel and support to residents with psychosocial needs, those self‑directing care that limits their functional level, and those needing additional support and TLC.

Provide summary documentation no less than quarterly based on the MDS schedule. The documentation shall be a progress note completed before the MDS is assigned.

Complete BIMs, Mood, Psychosocial, Behavior, and Discharge observations timely and in accordance with policies and guidelines.

Complete sections of the MDS as assigned on time and accurately.

Maintain daily documentation on resident/representative interactions.

Facilitate room changes.

Participate in QAPI by completing and presenting audits as assigned and identifying issues related to resident wellbeing.

Complete all required documentation in the medical record (EMR).

Work collaboratively with all disciplines, internally and externally, to ensure the highest quality of care for residents and families.

Other duties as assigned.

Qualifications:

Bachelors degree in Social Work, Human Services, or Psychology with experience in geriatrics.

Must have one year supervised social work experience (paid/volunteer) in a health‑care setting working directly with individuals.
