Summary
The Social worker assigned to the HBPC (Home Based Primary Care) will be responsible for Veterans and family needs in the home setting (this may be a private residence - Medical Foster Home - Assisted Living - or other area Veteran designates as his/her home). In addition to serving Veterans in HBPC, the incumbent may serve those Veterans who receive care under other REC areas based on work‑load demands.
Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education
Have a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Graduates of schools of social work that are in candidacy status do not meet this requirement until the school is fully accredited
A doctoral degree in social work may not be substituted for the master’s degree in social work
Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a master of social work
Licensure
Persons hired or reassigned to social worker positions in the GS‑0185 series must be licensed or certified by a state to independently practice social work at the master’s level

Exception: VHA may waive the licensure or certification requirement for persons who are otherwise qualified pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS‑9 grade level. For the GS‑11 grade level and above, the candidate must be licensed or certified. At the time of appointment, the supervisor will provide the unlicensed/uncertified social worker with written requirements for licensure or certification, including deadlines and consequences.
Failure to Obtain License or Certification
Social workers must actively pursue meeting state prerequisites for licensure or certification starting from the date of their appointment
Failure to become licensed or certified within three years will result in removal from the GS‑0185 series and may result in termination of employment
Once licensed or certified, social workers must maintain a full, valid, unrestricted license to remain qualified for employment. Loss of licensure will result in removal and possible termination
Driver’s License: A current state‑issued driver’s license is required. If an interview is requested, a copy of the current license must be provided.
English Language Proficiency: Candidates must be proficient in spoken and written English as authorized by 38 U.S.C. § 7403(f). Grandfathering provision may apply to current VHA employees.
Grade Determinations: Social Worker – GS‑9 Experience – Education – and Licensure. None beyond the basic requirements.
Demonstrated Knowledge‑Skills‑Abilities (KSAs)
Work with Veterans and families from diverse backgrounds using counseling skills
Assess psychosocial functioning and needs; formulate treatment plans
Implement treatment modalities for individuals, families, and groups
Use supportive, problem‑solving, or crisis intervention techniques
Establish and maintain working relationships and communicate with clients, staff, and community agencies
Fundamental knowledge of medical and mental health diagnoses, disabilities, treatment procedures, medications, and terminology
Social Worker – GS‑11 Experience and Licensure
Appointment requires a minimum of one year of post‑MSW experience in health care or related settings and licensure or certification at the independent practice level
Alternatively, a doctoral degree in social work may substitute for professional experience
Additional KSAs for GS‑11
Knowledge of community resources, referral skills, and service coordination
Independent psychosocial assessments and treatment interventions for diverse populations
Ability to provide consultation to new social workers and staff

Duties
Conduct comprehensive psychosocial assessments of veterans, families, and caregivers.
Active engagement in the treatment planning process with interdisciplinary team, ensuring psychosocial needs are considered.
Apply counseling skills to implement treatment plans, including interventions for disability, terminal illness, and death.
Provide consultation and education to veterans and families on community resources, advance directives, and VA benefits.
Serve as a consultant for teammates, providing insights and guidance.
Act as a liaison between VA and community resources; may perform marketing and public relation duties when needed.
Deliver case management services across the continuum of care, ensuring veterans and families receive support at every stage.
Offer wellness and prevention education.
Use computer software for documentation, data management, and tracking within VHA framework.
Maintain data and reports to comply with JCAHO, VA, and Medical Center policies.
Document social work interventions and activities in patient records for smooth hand‑off.
Establish and maintain effective relationships with patients, families, and community agencies.
Apply knowledge of medical and mental health diagnosis, especially acute, chronic, and traumatic conditions and medication effects.
Engage in professional growth and participate in peer reviews and case conferences.
Drive to community locations and transport veterans in government vehicles; valid driver’s license required for travel.

Work schedule: Monday‑Friday, 7:30 a.m.–4:00 p.m. or 8:00 a.m.–4:30 p.m.
Pay: Competitive salary and regular salary increases; step rate may be higher for extra qualifications.
Paid Time Off: 37‑50 days of annual paid time including 13‑26 days of annual leave, 13 days of sick leave, and 11 paid federal holidays. Credit for prior work or military service may apply.
