We are seeking a

Substance Use Disorder (SUD) Healthcare Navigator

to join our

New Day Recovery Program in Saltville, VA , supporting individuals navigating recovery from substance use and co-occurring mental health conditions.
This role is ideal for someone with a background in community health, case management, behavioral health, or peer recovery support who is passionate about improving access to care and addressing social determinants of health.
Position Summary

The Healthcare Navigator plays a critical role in guiding patients through the continuum of care by coordinating services, removing barriers, and connecting individuals to both internal SVCHS programs and external community resources.
This position works collaboratively within an interdisciplinary d requires strong relationship-building skills, patient advocacy, and knowledge of recovery-oriented systems of care.
Key Responsibilities

Coordinate access to Substance Use Disorder (SUD), behavioral health, and primary care services within SVCHS and the community
Develop, maintain, and strengthen referral pathways with community partners (housing, employment, mental health, social services)
Provide care navigation, follow-up, and patient engagement support to ensure continuity of care
Assist patients in overcoming barriers to care, including transportation, insurance, and social determinants of health
Support and educate individuals participating in peer recovery services
Maintain up-to-date knowledge of local and regional community resources
Document patient interactions and care coordination activities in accordance with organizational standards
Minimum Qualifications

Education:

High school diploma or equivalent (required)
Experience:

Experience in

community health, healthcare navigation, case management, behavioral health, or social services
Knowledge:

Understanding of

Substance Use Disorders (SUD), mental health conditions, and community-based resources
Licensure:

Valid driver's license with the ability to transport patients as needed
Skills:

Strong communication and interpersonal skills
Ability to build trust with diverse and vulnerable populations
Effective care coordination and problem-solving abilities
Team-oriented with the ability to work in a fast-paced environment
