Job Summary
The Nurse Case Manager (NCM) II reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager II serves as a clinical expert related to discharge planning, resource utilization, and case management.

Minimum Training and Education
Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required.
Prior leadership experience preferred.
One year of Case Management experience preferred.
Case Management certification preferred.
Required Licensure, Certifications, Registrations
Licensure as a registered nurse by the state of South Carolina or a compact state required.
Certification in Case Management preferred.
Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.
Physical Requirements
Ability to perform job functions while standing, sitting, walking, climbing stairs, working indoors and outdoors in temperature extremes, from elevated areas, and in confined spaces. The position requires frequent kneeling, twisting, squatting, pinching, gross motor, fine manipulation, overhead reaching, repetitive hand/wrist/elbow/shoulder motions, full use of both legs, lower extremity balance, and the ability to lift and carry 50 lbs. unassisted, lift/lower objects 50 lbs. from/to floor from/to 36 inches unassisted, lift from 36" to overhead 25 lbs., and exert up to 50 lbs. of force. Vision and hearing acuity, including corrected vision and hearing, are required. The position requires dealing with stressful situations, rotating shifts, overtime, latex safe environment, and selected respirator use.
