Licensed Social Worker (Per Diem/Temporary)
We are seeking a Licensed Social Worker for the South New Jersey area.
General Responsibilities
Responsible for providing a wide range of psychosocial services to terminally ill patients and their families, including comprehensive psychosocial assessment, ongoing assessment of patient and family psychosocial needs, bereavement risk assessment, counseling related to anticipatory grief, and developing and implementing psychosocial plans of care in collaboration with the Interdisciplinary Team.
Services may include supportive counseling, problem solving and referrals to community resources, pre‑bereavement counseling, and support.
Education/Training/Certifications
Master’s Degree in Social Work (MSW) from a graduate school accredited by the Council on Social Work Education (CSW) in accordance with N.J.S.A 45:15BB-1 et seq. and N.J.A.C 13:44G.
Current license as a Social Worker by the New Jersey State Board of Social Work Examiners.
Licensed driver with a vehicle in good working condition and required insurance in accordance with New Jersey laws.
Skills and Knowledge
Visual/hearing ability sufficient to comprehend written or verbal communication.
Excellent verbal communication skills.
Ability to manage stress effectively.
Manual dexterity sufficient for writing, keyboarding, and telephone operation.
Ability to work in a variety of settings and conditions in patients’ homes and facilities.
Experience
Understanding of hospice philosophy and principles of death and dying.
Hospice or home care experience desired.
Ability to assess patient needs and develop individualized care plans.
Ability to work in a field setting and exercise sound judgment.
Strong communication, comprehension, documentation, interpersonal and computer skills.
