Summary of Job Duties

The Registered Nurse will promote wellness, improve outcomes, and support self‑care within a defined patient population. The RN will provide patients with chronic conditions the support, education, and assistance to prevent and maintain their disease and overall health and wellness state. The role involves developing and implementing individual health plans, coordinating outpatient care, managing prescription refills and patient education, and committing to high levels of patient outcomes despite workload changes.
Qualifications

Minimum Qualifications

Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing.
Valid RN License.
Minimum of 1 year Registered Nursing experience.
Preferred Qualifications

3 years of RN experience with current BLS certification.
Knowledge or experience in PCMH.
ACLS certification or obtain certification within one year of hire date.
Responsibilities

Collect and document assessment data from patients and/or others per UAMS standard.
Develop patient’s Plan of Care (POC) per department/unit standards using assessment data.
Evaluate patient progress toward desired outcomes.
Administer medication following department/unit standards and the Nurse Practice Act.
Verify and evaluate care to ensure consistency with patient and family‑centered care principles, practice standards, and regulatory requirements.
Support integration of current evidence into nursing practices.
Promote an environment conducive to positive educational experiences for all students and staff.
Provide effective patient/family education and discharge planning.
Collaborate and communicate effectively with nursing staff, interdisciplinary teams, executive leaders, and stakeholders.
Document care appropriately in the medical record.
Report variances in care, system, or process through appropriate channels.
Participate in staff recruitment and retention efforts.
Facilitate development and implementation of evidence‑based nursing practice standards in compliance with ANA standards.
Perform other duties as assigned.
