Case Manager/Care Manager Full-Time (FT) - $8,000 Sign-On Bonus!

Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI. Offering an $8,000 sign‑on bonus! This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

Case Manager/Care Manager Job Responsibilities

Manage caseload of Family Care Program members

Complete comprehensive clinical assessments

Work collaboratively to develop member‑centered care plans, provide ongoing care coordination, member education, quality assurance, and monitor services

Travel: regular local travel requiring use of personal vehicle, valid driver’s license and proof of required insurance coverage

Case Manager/Care Manager Job Requirements

Education: Bachelor’s Degree in Human Services or closely related field required.

Licensure: Current long‑term care functional screener certification preferred (eligibility for this certification required).

Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities required.

3 years of work in community nursing preferred.
