Position Overview
Psychiatric Mental Health Nurse Practitioner (PMHNP) – PT Onsite, PMHNP for ages 15 and up, predominately adults.
Schedule
Mon/Wed/Fri 8:00 AM–5:00 PM.
Shifts
PMHNP/FNP – Crisis Sobriety Center – AM/PM shifts (Sunday–Tuesday rotating) (Rotating Wed/Thursday‑Saturday).
PMHNP – SMI Caseload – Full time on site, Mesa location; 8:00 AM‑4:00 PM Monday–Friday.
PMHNP – ACT/FACT – Full time on site, Central Phoenix 8:00 AM‑5:00 PM Monday–Friday.
Responsibilities
Complete urgent psychiatric and/or addiction medicine evaluations for patients referred to the 23‑hour observation unit.
Perform urgent psychiatric assessments and/or addiction medicine, ambulatory detox, or physical health assessments as identified by screening staff.
Coordinate admissions, transfers, and discharges, documenting transitions in the medical record.
Initiate and document direct‑to‑doctor communication and care coordination.
Provide daily evaluations for all patients on the unit, adjusting treatment and discharge planning as needed.
Implement seclusion and restraint interventions in accordance with CBI policy, documenting assessments, orders, durations, and releases.
Review cases and discharge plans during rounds, and sign full discharge orders and follow‑up recommendations within 24 hours of discharge.
Complete application for involuntary evaluation and application for emergency admission within two hours of receipt, notifying the Admission Coordinator and, if necessary, the Medical Director.
Maintain patient‑centered, evidence‑based care, serving as an advocate for patients across all health‑related matters.
Participate in quality management initiatives, addressing identified deficiencies within 72 hours.
Coordinate internal consults, pain management evaluations, and care for patients under petition or court‑ordered treatment.
Document and sign full discharge orders and assess ongoing mental and physical health care needs for each patient.
Maintain minimum 85% compliance with documentation requirements and a 75% follow‑up rate.
Carry out required training, education meetings, and standing rounds as scheduled.
Qualifications & Requirements
Licensed psych‑mental health nurse practitioner with experience in inpatient and outpatient settings.
Suboxone waiver to treat the maximum number of eligible patients.
Current CPR and First Aid certification, renewed every two years.
TB skin test completed prior to patient contact, with annual updates.
Proficiency with electronic health record and e‑prescribing systems.
Ability to maintain a 75–85% patient satisfaction rating and minimum 60‑day notice for time off.
Universal Requirements
Complete orientation, shadowing, and TB screening as directed by CBI.
Maintain suboxone waiver, patient data logs, and medication assessments.
Participate in quality management and correction of deficiencies within 72 hours.
Adhere to scheduling protocols, appointment cancellations, and patient appointments as directed.
Provide evidence of CPR/First Aid and documentation of renewal to Credentialing department.
Support utilization review and management activities, documenting admission and continued stay necessity.
Maintain collaboration with referral and receiving professionals, and participation in practitioner meetings and peer review.
