Wellness LPN or LVN Nurse

Brookdale offers a role focused on delivering compassionate care and continuous support to residents. The caregiver will assist in maintaining a physical, social, and psychological environment that aligns with the residents’ best interests. They will monitor and assess residents’ health and wellbeing, administer prescribed treatment plans, coordinate with providers and pharmacies, and, for nurses, monitor treatment responses and report any issues to the resident care coordinator or executive director.
Base pay will be determined by applicant’s skills and experience. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities
Assist in maintaining a physical, social and psychological environment in the best interest of residents.
Monitor and assess residents’ health and wellbeing while administering treatment plans prescribed by a physician. Coordinate with providers and pharmacies to ensure continuity of care.
Nurses also monitor responses to treatment plans and notifies the resident care coordinator or executive director of any issues.
LPN or LVN License Required per state regulations.
