RN - Hospice House Registered Nurse

Take charge of developing, implementing, and managing personalized patient care plans in a hospice house setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes. Deliver compassionate, respectful care while fostering strong relationships with patients, families, and colleagues.

Essential Functions
  • Create individualized care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics.
  • Provide skilled nursing services aligned with patient and family goals, focusing on symptom management and comfort.
  • Communicate patient assessments and care plans regularly with hospice house staff and volunteers.
  • Work collaboratively with physicians, patients, families, and other disciplines to update care plans as needed.
  • Supervise Home Health Aide services according to policy.
  • Maintain timely and accurate documentation following agency guidelines.
  • Build and sustain positive working relationships with patients, families, co-workers, physicians, and referral sources.
  • Observe and assess patients' physical, mental, emotional, and spiritual conditions, coordinating with team members appropriately.
  • Adhere strictly to HIPAA regulations regarding patient privacy and information security.
Minimum Knowledge and Skills
  • Proficiency with MS Office and keyboarding skills.
  • Effective communication abilities.
  • Strong multitasking and prioritization skills.
  • Application of fundamental nursing principles.
Preferred Knowledge and Skills
  • Clinical nursing skills relevant to hospice care.

Education and Experience
  • Graduate of an accredited nursing program.
  • Several years of clinical nursing experience.
  • Preferred: Bachelor of Science in Nursing (BSN).
  • Preferred: Previous hospice and/or home health experience.
  • Preferred: Certification as a Hospice and Palliative Care Nurse.
Licenses and Certifications
  • Current Registered Nurse license in the relevant state.
  • BLS certification required within three months of hire.
  • Valid driver's license and proof of auto liability insurance as required.
