As the Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment you will work to determine a treatment plan, perform interventions to improve the patient's well-being, and evaluate progress.
Primary Responsibilities:
Evaluate the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with the health‑care team.
Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician.
Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
Instruct participants of the health‑care team, the patient, and caregivers regarding the plan of care, functional limitations, and progress toward goals.
Visit patients according to the Plan of Treatment, complete a progress note for each visit, and submit notes per LHC policy.
Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.
Report outcomes of evaluation, goals, and anticipated projected frequency of care.

Required Qualifications:
Current Physical Therapy licensure in the state of practice.
Current CPR certification.
Current driver’s license and vehicle insurance, or reliable vehicle or public transportation.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Pay Range
$44.65 – $98.65 per visit point.
Annual total cash compensation for full‑time employment (40 hours per week) follows the above range and includes earnings from per‑visit point pay and hourly pay. Compensation varies by local labor markets, education, work experience, and may increase over time based on productivity and performance. You receive per‑visit pay for visits, and hourly pay for non‑visit activities such as orientation. The company complies with all minimum wage laws.
