
Physical Therapist- Sign on Bonus

As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain and other medical conditions. Based on this assessment and evaluation you will help determine a treatment plan, perform interventions aimed at improving and enhancing the patient’s well-being, and evaluate the patient’s progress.
Primary Responsibilities
Evaluate the patient’s functional status and physical therapy needs; assist the physician in the development of the therapy plan of care.
Provide services within the scope of practice as defined by state laws in accordance with the plan of care and in coordination with other members of the health‑care team.
Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or the physician.
Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
Instruct and inform the patient, family or caregivers, and other health‑care team members regarding the plan of care, functional limitations, and progress toward goals.
Visit patients according to the plan of treatment, complete a progress note for each visit, and submit progress notes per LHC policy.
Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.
Report outcomes of evaluation, goals, and anticipated projected frequency of care.
Sign On Bonus – Eligible Full‑Time Candidates
Eligible full‑time candidates will receive a sign‑on bonus as part of their compensation package.
Required Qualifications
Current Physical Therapy licensure in the state of Oregon.
Current CPR certification.
Current driver’s license and vehicle insurance or reliable vehicle access; or public transportation.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Pay is based on several factors including local labor markets, education, work experience, certifications, and more. The hourly pay for this role ranges from $34.23 to $61.15 per hour for full‑time employment. In addition to salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (subject to eligibility requirements).
