In this role, you will provide comprehensive primary care to patients, addressing a broad range of medical concerns and supporting the health of the community.

Health promotion
Disease prevention
Health maintenance
Counseling
Patient education
Annual Health Assessments (AHA)
Chronic Care Management (CCM)
Provide direct and indirect patient care, identifying risks via screening.
Collaborate with providers, ancillary personnel, and interdisciplinary teams to resolve patient needs.
Leverage Electronic Health Record (EHR) to manage and improve patient care, workflows and compliance.
Advocate for patients to achieve cost‑effective care.
Promote effective communication and encourage patient partnership.
Qualifications & Experience
Minimum 1‑2 years of clinical experience.
Successful completion of residency in the specialty.
Preferred: 3+ years as a primary care provider.
Significant experience with an EHR, preferably Epic.
Active, in‑good‑standing California Medical Licensure and Board Certification.
Active, good‑standing NPI number.
Active, good‑standing DEA number (US Drug Enforcement).
Ability to function independently and in a team in a dynamic environment.
Excellent communication skills and adaptability.
Highly organized with strong time‑management and flexibility.
Experience with evidence‑based practice and quality improvement.
Compensation & Benefits
The base salary for this position is $225,000–$255,000 per year.
