PURPOSE AND SCOPE
Under the general supervision of the Manager - HUB Mental Health and Wellness Services, the Case Manager works with a multi‑disciplinary team to provide clinical case management, outreach services and works with internal and external stakeholders to coordinate resources and support to improve the mental health and overall wellness of LSC students, faculty, and staff. Works in a highly collaborative environment that values teamwork, flexibility, and adaptability. Delivers direct clinical services, including case management, psychoeducation, triage, brief counseling, risk assessment, crisis intervention, referrals and outreach to the campus community of students, faculty and staff. May provide crisis and emergency response during after‑hours. Plays a vital role in campus‑wide mental health and wellness initiatives to the campus community. Moderate travel between LSC locations is required. Works a flexible schedule (evenings and weekends) to provide after‑hours clinical services and crisis coverage.
ESSENTIAL JOB FUNCTIONS
Provide triage services for students seeking mental health support, including case management, psychoeducation, brief personal and career counseling, risk assessment, crisis intervention, and referrals to on‑ and off‑campus services.
Develop, foster, and maintain collaborative relationships with on campus and off campus resources to ensure students are connected to appropriate services and resources.
Plan and implement outreach programming, workshops, and consultation services for students, faculty, staff, and the broader community.
Actively assist students in applying for local, state, and federal benefits.
Consult with parents, faculty, and staff when appropriate and with proper authorization to ensure continuity of care.
Support students during the appointment check‑in process, including monitoring and managing check‑in systems.
Utilize the electronic health record (EHR) system and other LSC platforms to document client interactions in compliance with legal, ethical, and professional standards.
Participate in multidisciplinary team meetings.
Work a flexible schedule to accommodate after‑hours clinical services and crisis response coverage.
Responsible for other reasonable related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
Possesses strong knowledge of college student development and mental health issues, with sensitivity to multicultural concerns and understanding of relevant laws and regulations.
Excellent customer service skills.
Able to work evening/weekend outreach and emergency support as needed.
Skills in coordinating, planning, and implementing targeted campus events and activities.
PHYSICAL ABILITIES
The work is sedentary. Typically, the employee sits comfortably to do the work. However, there may be some walking; standing; bending; carrying of light items, such as papers, books, or small parts; or driving an automobile. No special physical demands are required to perform the work.
WORK SCHEDULE AND CONDITIONS
Equipment used includes PC workstation running on a LAN in a Microsoft Windows environment, calculator, phone, and other general office equipment.
Interface with internal and external contacts as needed to carry out the functions of the position.
Work is performed in a climate‑controlled office with minimal exposure to safety hazards.
REQUIRED QUALIFICATIONS
Bachelor's degree and at least 3 years of related experience, or an equivalent combination of education and experience.
PREFERRED QUALIFICATIONS
Master's degree without license or exam, or;
LPC, LCSW, LPA‑I, or LMFT.
Salary
Hiring salary range is $48,152-$55,375.
