Job Description

Job Title: Wellness Self Management Specialist - Psychiatry Administration - Rivington - Full Time - Day

The purpose of Assertive Community Treatment (ACT) is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met in the traditional outpatient setting. ACT is an evidence-based practice providing psychiatric treatment, case management, and support services delivered by a multi-disciplinary mental health treatment team.
The Wellness Self-Management Specialist will work with other ACT treatment team members to engage clients in treatment, identify person centered goals, and empower clients to develop life skills. The Wellness Self-Management Specialist provides services to individuals and in group settings. This is a field-based position and requires community outreach visits with clients in their residences, shelters, etc. to ensure continued engagement treatment.
Qualifications

Master?s Degree in Psychology, Social Work, or related discipline preferred. Applicable professional license in field issued by NYS Education Department strongly preferred.

Experience working with individuals with serious mental illness and co-occurring substance use disorders.

Ability to travel via public transit independently, or with reasonable accommodations, to provide community outreach.

Qualification as New York State Office of Mental Health professional clinical staff (Master?s degree and/or licensure in Social Work, Psychology, Occupational Therapy, Rehabilitation Counseling, Pastoral Counseling, Creative Arts Therapy, Therapeutic Recreation) preferred.

Responsibilities

Clinical/Technical/Service

Assume primary case management responsibility for 8-12 adults with serious and persistent mental illness.

Meets with clients individually to establish realistic treatment goals, prioritizing person centered goals and reviewing goal attainment regularly.

Provides individual and/or group training in healthful living, self-management of chronic mental illness, well living skills, creation of healthful lifestyles, and support in areas such as self-care, holistic wellness, hygiene, and other life skills as needed.

Attend and participate in daily morning meetings with ACT team members to discuss clients’ clinical and case management needs; Collaborates with team members to develop and implement treatment plans involving multiple modalities.

Completes Comprehensive Assessments, Treatment Plans, and other documentation as required within a timely manner.

Complete documentation and reporting as required by the NYS Office of Mental Health (OMH) and NYC Department of Health and Mental Hygiene’s (DOHMH) guidelines.

Organizational/Managerial

Participates in the department’s performance improvement activities.

Maintains patient/employee confidentiality in the management of information.

Observes the Health Care System’s compliance policies.

Educational/Professional Development

Meets regulatory, licensure, and annual health assessment requirements.

Identifies learning strengths
eeds and utilizes resources for continuing education.

Communication/Relationships

Demonstrates a professional, courteous, and respectful attitude in dealing with clients, families, and collateral contacts.

Displays courtesy, tact, and patience during interactions with all members of the hospital staff and extended community.

May be required to perform other duties as assigned or perform other duties when necessary.

Compensation Statement

Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661 - $67500 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
