The FDVA Nursing State Veterans’ Nursing Homes and Domiciliary are designated employment sites for the Nursing Student Loan Forgiveness Program. From funds available, the program may make nursing education loan repayments of up to $4,000.00 per year, per nurse enrolled in the program, for a maximum of four years. Payment is made at the end of each 12-month enrollment period and made directly to the participant’s lender. Enrollment and payment of awards is based on available funds.
Minimum Qualifications
Florida State Licensed Registered Nurse.
Six (6) years of professional nursing experience. A bachelor’s degree from an accredited college or university with a major in nursing or a related field AND two years of nursing experience or a master’s degree from an accredited college or university in nursing, nursing education, public health, or a related field may substitute for the nursing experience.
Must have MDS Certification and have experience with MDS Care Plan.
Must have excellent computer skills.
Must be willing to work weekends or holidays if necessary
Successful completion of the due diligence process to include, but not limited to, a Level II background check and drug test.
Preferred Qualifications
Experience in a long-term health care facility.
Position Description
The incumbent will be responsible for scheduling or overseeing scheduling of all resident MDS assessments to ensure timeliness.
Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements.
Participate in the pre-admission process to determine potential residents’ level of care.
Participate in resident level of care determination upon admission.
Participate in tracking changes in residents’ level of care to ensure timely scheduling of MDS assessments.
Be familiar with residents’ condition and care needs.
Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as necessary.
Participate in the Quality Assessment and Assurance Program, in cooperation with the Quality Assurance Coordinator.
Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure this information is included.
Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
Maintain confidentiality of resident and facility information.
Complete reports and assignments accurately and adhere to established time schedules.
Establish open lines of communication between all departments.
Observe infection control procedures relating to nursing services.
Observe all facility safety policies and procedures.
Come to work as scheduled and consistently demonstrate dependability and punctuality.
Consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families and consultants.
Perform other duties as assigned by the Director of Nursing Service.
Perform all duties assigned in a cooperative, effective, timely and professional manner.
