Responsibilities:

Overall responsibility for developing new referral relationship opportunities for perspective hospice patients; acting as a resource to these accounts and existing accounts in an assigned territory.

Qualifications:

Graduate from an accredited School of Socia Work MSW.
Experience in Hospice or Palliative Care preferred and experience in Account Management.
Knowledge of COPs for Hospice and Palliative Care and reimbursement for Hospice desirable.
Minimum of one-year experience in outreach, sales or marketing.
CHPN after first year of hire.
Ability to work within an interdisciplinary team environment
Excellent public speaking abilities.
Well-developed teaching, public speaking, leadership, and organizational skills
Able to work creatively within budgetary constraints
Goal Oriented Change Agent
Excellent verbal and written communication skills
Advanced program development skills
