Position Overview
The Community Resource Coordinator is responsible for coordinating discharge planning, reentry, and reintegration services for individuals releasing from the facility. This position works in collaboration with a multidisciplinary treatment team to identify needs, develop transition plans, coordinate referrals, and support continuity of care upon release. The position assists individuals with linkage to community-based services, behavioral health and substance use supports, MAT‑related discharge planning, housing resources, identification documents, benefits, transportation, and other stabilization needs. This role will assist with reentry-focused programming and groups, depending on facility needs and program capacity.
Schedule
Monday‑Friday, 8:00 a.m. – 4:00 p.m.
Essential Job Duties
Interviews inmates to provide preliminary discharge plans
Identifies who needs outside resources upon release
Electronically documents all interventions and patient responses in the medical record
Assists in matching referrals to needed services
Assists in filling out required or relevant paperwork
Provides patients with information needed for aftercare services
Serves as liaison between County Alcohol, Drug, and Mental Health Office, County Public Defender's Office, and the Jail Mental and Medical units
Participates in case management meetings with on‑site mental health providers
Participates in Quality Management meetings, Quality Management Peer Review activities, and staff training
Responsible for discharge planning for MAT patients in accordance with policy and procedures
Plans, develops, and recommends priorities, goals, and long‑range objectives pertaining to the management and direction of the detention facility
Collects and analyzes program and community data to assess community strengths and needs
Identifies participants and community partners to meet patient needs and maximize use of resources
Responsible for providing individual and/or group treatment for identified MAT patients scheduled for release
Assists with individual or group reentry/reintegration programming under clinical oversight when applicable
Assists with developing program brochures and other related marketing items
Performs other duties as assigned
Minimum Education and Experience
Associate’s degree in a health‑related, human services, criminal justice, social work, psychology, counseling, or related field preferred
Bachelor’s or master’s degree in social work, counseling, psychology, criminal justice, human services, or related field preferred
Two to five years of case management, social services, correctional, behavioral health, substance use, or reentry experience required
Correctional, Detention, Jail, or Community Reentry experience required
Experience working with justice‑involved individuals, high‑needs populations, behavioral health, substance use, or community‑based service coordination experience required
A combination of education and experience will be reviewed on a case‑by‑case basis
Preferred Credentials
Certified Alcohol and Drug Counselor certification preferred
Certified Peer Support Worker certification preferred
Additional training in motivational interviewing, trauma‑informed care, evidence‑based reentry practices, MAT/MOUD, CBT‑based programming, or case management preferred

Employment Requirements
Must maintain all certifications, educational requirements, licensing, etc.
Must have current CPR/BLS certification
Must have current TB test within one year
Must adhere to all facility policies and procedures
Willing to assist coworkers and work overtime if required; may act as a resource to other coworkers
Maintains confidentiality, dignity, and security of health records and protected health information in compliance with HIPAA requirements
Security Requirements
Must undergo security training and orientation on facility safety policies and procedures
Physical/Mental Requirements
Ability to remain stationary and move/traverse facility, including stairs
Ability to position oneself in different spaces
Ability to convey and discern information in conversations, frequently communicate with patients, and exchange accurate information
Ability to identify and detect objects and assess situations from a variety of distances
Ability to remain calm in stressful and demanding situations
Work Environment
Work is performed inside a correctional facility. Employees are exposed to risk and/or harm by inmates, including exposure to blood‑borne pathogens. Employee is expected to comply with all security rules and regulations to minimize risk.
Other
Employee must comply with all current and future State, Federal, and Local laws and regulations, court orders, administrative directives and standards, and policies and procedures of the site where assigned, including those of professional organizations such as ACA, NCCHC, etc. Employee must treat every other member of the CorrHealth team, all correctional personnel, all inmates and third parties in the facility with proper dignity and respect. Actions or communications that are inappropriate or degrading will not be tolerated.
Must be able to pass a background check and pre‑employment drug test.
