
Community Health Nurse

The Department of Health (DOH), Bureau of Community Health Systems is seeking a Community Health Nurse to play a key part in improving health across Pennsylvania communities. This role supports public health efforts and works directly with local partners to bring essential services to those who need them most.
Description of Work
Public Health Assessment: Analyze health data to identify needs and guide service planning.
Community Collaboration: Work with partners to support programs and strengthen public health efforts.
Health Education: Provide programs and resources that help the public understand key health topics.
Clinical Services: Deliver immunizations, screenings, and other services that protect community health.
Disease Monitoring: Investigate illnesses and coordinate efforts to prevent the spread of disease.
Outreach Support: Assist individuals and groups in accessing needed public health services.
Work Schedule and Additional Information
Full‑time employment, 37.5 hours per week.
Work hours: 8:00 AM to 4:00 PM, Monday – Friday, with a 30‑minute lunch.
Irregular schedule: Requires statewide travel and irregular, flexible work hours including overnights, weekends, and holidays in accordance with policy.
Telework: This position does not allow telework.
Salary: In some cases, the starting salary may be non‑negotiable.
Special Requirements
Must possess active authorization to practice as a Registered Nurse in Pennsylvania.
All employees with an active temporary practice permit must obtain licensure as a Registered Nurse within the one‑year period defined by the Pennsylvania State Board of Nursing.
Other Requirements
Must meet the PA residency requirement.
Must be able to perform essential job functions.
Legal Requirements
Must pass a background investigation.
This position falls under the provisions of the Child Protective Services Law.
Under the law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
