
Community Health Nurse

Overview
The Department of Health (DOH), Bureau of Community Health Systems, is seeking a Community Health Nurse to improve health across Pennsylvania communities. This role supports public health work by assessing needs, planning services, and helping improve overall health outcomes. The nurse will work with partners, provide clinical services, and support disease prevention efforts.
Responsibilities
Public Health Assessment : Analyze health data to identify needs and guide service planning.
Community Collaboration : Work with partners to support programs and strengthen public health efforts.
Health Education : Provide programs and resources to help the public understand key health topics.
Clinical Services : Deliver immunizations, screenings, and other services that protect community health.
Disease Monitoring : Investigate illnesses and coordinate efforts to prevent the spread of disease.
Outreach Support : Assist individuals and groups in accessing needed public health services.
Qualifications
Active authorization to practice as a Registered Nurse in Pennsylvania.
Employees with a temporary practice permit must obtain licensure as a Registered Nurse within one year as defined by the Pennsylvania State Board of Nursing.
Must meet Pennsylvania residency requirement.
Must be able to perform essential job functions.
Must pass a background investigation.
Legal Requirements
Background investigation required.
This position falls under the provisions of the Child Protective Services Law, requiring satisfactory criminal history reports including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
Work Schedule and Additional Information
Full-time employment, 37.5 hours per week. Work hours are 8:00 AM to 4:00 PM, Monday through Friday, with a 30‑minute lunch break. The position requires infrequent statewide travel and irregular, flexible work hours including overnights, weekends, and holidays. Telework is not an option. Salary may be non‑negotiable in some cases.
