She Recruits is partnering with a well-established Federally Qualified Health Center (FQHC) in Brooklyn, NY seeking a Family Nurse Practitioner to join their mission-driven primary care team.

The Family Nurse Practitioner will provide comprehensive primary and preventive care across the lifespan in a fully outpatient setting.
You will work alongside physicians, nurses, medical assistants, behavioral health professionals, and care coordinators in a structured, collaborative environment designed to support high-quality patient care.
Key ResponsibilitiesProvide comprehensive primary care for pediatric, adult, and geriatric patients
Diagnose and manage acute and chronic conditions including diabetes, hypertension, asthma, and common illnesses
Perform preventive screenings and wellness exams
Develop individualized treatment plans
Prescribe medications per New York State guidelines
Provide patient education on disease prevention and lifestyle modification
Maintain accurate, compliant EMR documentation
Coordinate care with specialists and community resources
Participate in quality improvement initiatives aligned with FQHC standards
Active and unrestricted New York Nurse Practitioner license
DEA license (or eligibility to obtain)
Family Medicine or Primary Care experience preferred
Experience serving underserved populations strongly valued
Strong clinical judgment and ability to work independently within a team model
Bilingual skills (Spanish or other languages) a plus

Comfortable working with medically and socially complex populations
