Job Summary
Acts is currently seeking qualified candidates for our Physical Therapist position. The role involves providing physical therapy services to residents in a welcoming, slower‑paced community environment that focuses on prevention and holistic care.
Responsibilities
Conduct evaluations and develop individualized treatment plans and goals.
Administer treatments, discharge planning, and other related duties as required.
Follow residents throughout the full continuum of care, including inpatient, home health, outpatient, and fitness services.
Qualifications
Bachelor’s degree in Physical Therapy.
Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered.
Experience in a skilled nursing and/or outpatient rehabilitation environment (preferred).
