Mental Health Case Manager

Telework Eligible: Yes. Applicants must reside in Minnesota or a bordering state, within 50 miles of the primary work location to be eligible for telework.

Responsibilities

Provides a combination of direct and indirect case management and functional support services to deaf, deafblind, and hard‑of‑hearing clients within the State Services’ Mental Health Program.
Works collaboratively with the State Services team and other providers to ensure effective and coordinated care.
Maintains accurate, timely, and complete client documentation in accordance with program and state requirements.
Connects clients to external resources and services to support holistic care and recovery.
Ensures that all communications regarding their deaf, deafblind, and hard‑of‑hearing clients are handled promptly and efficiently.
Ensures all services are culturally and linguistically appropriate for deaf, deafblind, and hard‑of‑hearing clients.
Minimum Qualifications

1 year professional experience in case management and client support OR 3 years advanced technical or paraprofessional experience.
OR Bachelor’s degree in a related field and 2 years of advanced technical or paraprofessional experience.
AND experience must demonstrate the following:

Fluency in American Sign Language (ASL) and a strong understanding of Deaf, DeafBlind, Hard‑of‑Hearing, and hearing cultures, with the ability to effectively bridge communication and cultural differences.
Knowledge of mental health systems and practices, including assessment, case management, treatment planning, and crisis intervention for individuals who are Deaf, DeafBlind, or Hard‑of‑Hearing.
Understanding of applicable federal and state laws, rules, and policies related to mental health and disability services.

Preferred Qualifications

Experience collaborating with multidisciplinary teams and community partners to coordinate care and ensure access to appropriate services and accommodations.
Strong organizational and professional skills, including problem‑solving, time management, conflict resolution, and the ability to maintain accurate and timely documentation using computer systems.
Variety of experiences working effectively with others from different backgrounds and/or cultures.
