Brief Job Description
This position is available for Morning, Evening and Night shifts on multiple homes and may be eligible for additional pay for work performed on evenings, nights, weekends, holidays, and maximum security locations.

The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or have behavioral problems. The centers provide campus‑based direct services and supports at 13 centers located throughout the state.

As a Direct Care RN you will perform routine level nursing work, including assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. You will evaluate, treat and document healthcare needs and/or issues, and report them to the appropriate persons. You will communicate with direct care staff, physicians/PCP, administration, and other nursing staff as part of the interdisciplinary team, notifying the supervisor and PCP of trends and concerns to maintain consistent, high‑quality care. You will complete data entry, required documentation and reports and maintain records.

You will assist LVNs and Certified Medication Aides (CMAs) as needed, provide recommendations for improvement, participate in facility quality assurance/quality improvement (QA/QI) initiatives, and provide treatment to HHS employees injured in the course and scope of employment. You will respond to emergency situations to ensure safety and care of individuals and staff, working under moderate supervision with limited latitude for initiative and independent judgment.

Essential Job Functions

Attend work on a regular basis and perform other duties as assigned, including possibly rotating schedules, extended shifts, and overtime.

Serve in an on‑call status on a rotating schedule as required.

Perform routine nursing tasks and procedures in accordance with HHSC, SSLC and state regulations, such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments.

Collect specimens for laboratory examination and conduct appropriate screening tests when requested by lab personnel.

Administer medications, immunizations, and treatments, and observe patients for unusual symptoms and reactions.

Make rounds and/or home visits as needed and work in other locations at the SSLC as required.

Monitor individuals’ weight, diet, feedings and vital signs; monitor for signs of illness, and use appropriate nursing judgment to provide adequate care, including initiating acute care plans.

Assist in evaluating nursing intervention outcomes and provide input to other members of the healthcare team, including recommendations for improvements.

Instruct direct support professionals on monitoring after a treatment or procedure using instruction sheets (e.g., post‑sedation monitoring).

Document and report pertinent treatments and observations accurately and timely in the medical record (electronic or paper), in accordance with SSLC procedures.

Assist LVNs and CMAs with implementation of protocols, procedures and programs to improve health and safety of staff and individuals.

Assist with completing follow‑up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and medication variances.

Provide support to the Nurse Manager, including review of controlled drug counts, medication counts, emergency checklist logs, refrigerator temperature logs, and glucose controls.

Assist the Infection Control nurse with preventive measures and the Quality Assurance Nurse with therapeutic nursing programs.

Respond to emergency situations to ensure safety of individuals.

Provide educational and safety training to SSLC staff upon request.

Provide treatment to HHS employees injured while in the course of employment.

Perform other duties as assigned, including disaster response, recovery or Continuity of Operations (COOP) activation, which may require alternate shift patterns or locations.

Knowledge, Skills and Abilities

Knowledge of nursing techniques and procedures; of health care laws and regulations; of medical diagnoses, procedures and accepted medical treatment programs; and of community health and nursing care principles, practices and procedures.

Knowledge of state and federal laws and Intermediate Care Facility for individuals with intellectual disabilities (ICF‑IID) rules, regulations, policy and procedures related to State Supported Living Center, or the ability to acquire this knowledge quickly.

Knowledge of nursing care standards, the Settlement Agreement and the Healthcare Guidelines, or the ability to acquire this knowledge quickly.

Skill in the care and treatment of patients.

Skill in the use of computer/software and an electronic health record system.

Excellent verbal and written communication skills and the ability to present ideas clearly and concisely.

Ability to provide patient care, prepare and maintain records, interpret policies and procedures and healthcare regulations, and recognize patterns of medical necessity treatment.

Ability to demonstrate a working knowledge of emergency procedures and emergency equipment.

Ability to appropriately interact with individuals with intellectual/developmental disabilities, family members and staff.

Ability to work independently and productively with all levels of staff.

Ability to provide support and coaching to LVNs, CMAs and DSPs.

Ability to make decisions and recommendations to SSLC leadership, including on‑call administrators.

Registrations, Licensure Requirements or Certifications
Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Initial Screening Criteria

Work experience as a practicing registered nurse is required.

At least six (6) months of experience as a practicing registered nurse in a health agency, hospital, or health‑care facility is preferred.

Bachelor's Degree is preferred.

Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide, and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE). Flexibility in work hours may be required; overtime, extended hours, weekends and holidays may be requested. Candidates must be at least 18 years of age to be considered for employment at a state‑operated facility.
