
Licensed Practical Nurse - PACE

Position Summary
The Registered Nurse/Ambulatory is a member of the multidisciplinary team working in collaboration with physicians. The RN utilizes the care process consistent with their license or certification, initiating interventions within their scope of practice. The RN is responsible for coordinating the multidisciplinary plan of care and ensuring its consistency with the medical treatment plan. The patient care process includes:
Assessment – an orderly process of identifying, gathering, and organizing subjective and objective data pertaining to the patient/family/significant other.
Planning – identifying actual or potential problems and desired outcomes (goals) and documenting a plan of care that provides direction for the patient/family/significant other.
Intervention – implementing specific interventions or orders consistent with the plan of care and medical treatment plan.
Evaluation – determining the effectiveness of interventions, documenting required actions, and deciding on reassessment or revisions. Assessment and re-assessment identify problems prioritized by biophysical, psychological, environmental, self-care, educational, and social status; goals are consistent with problems to provide direction for patient care and health promotion; education is aligned with problems and goals to empower patients.
The RN assumes responsibility for continued professional and personal growth, supports organizational goals by providing quality customer service, participating in performance improvement efforts, and demonstrating a commitment to teamwork and cooperation.
Qualifications
Education:
Graduate of an accredited school of Nursing required. Bachelor Degree in Nursing preferred.
Licensure:
Current RN license in New Jersey required. Valid NJ Driver's License required. Proof of registration and insurance required. BLSHCP required.
Experience:
Minimum 1 year working with frail or elderly; minimum 1 year as a Home Health or Long Term Care Nurse or equivalent; ability to perform diversified duties with high accuracy; proficiency in problem solving, critical thinking, and priority setting; organizational skills.
Demonstrates competencies per the Assessment and Evaluation Tool.

Work Environment
Potential exposure to hazards in a hospital environment, blood borne pathogens, infectious disease, hazardous substances, and potential injury. The position involves considerable reaching, stooping, kneeling, crouching, extensive standing, sitting, walking; lifting approximately 25-50 pounds frequently and 50-100 pounds occasionally with assistance; periodic traveling to company off‑site locations; heavy mental stress and handling large numbers of interactions with multiple individuals amid constant interruptions. The essential functions are listed on the Assessment and Evaluation Tool.
Reporting Relationship
This position reports to department leadership and supervises incumbents as assigned.
