Responsibilities
Conduct comprehensive occupational therapy evaluations for residents to determine needs and goals.
Develop and implement individualized OT treatment plans focusing on activities of daily living, fine motor skills, upper‑extremity function, cognition, and safety.
Provide direct patient therapy sessions, document progress notes, update care plans, and discharge planning as appropriate.
Collaborate with an interdisciplinary team (nursing, physical therapy, speech‑language pathology, social services) and family/caregivers to optimize resident outcomes.
Re‑evaluate residents regularly and adjust treatment plans; support unit staffing by floating as needed within the facility.
Ensure compliance with documentation standards and payer requirements (e.g., Medicare/Medicaid) and maintain timely, accurate records.
Requirements
Active Occupational Therapy license in New Hampshire or eligibility to practice in NH (per state requirements).
Ability to work in a skilled nursing facility and float between units as needed.
Relevant OT experience in geriatric or long‑term care settings; travel experience preferred.
Strong clinical assessment and treatment planning skills; proficient in written and electronic documentation.
Effective communication and teamwork skills; ability to interact with residents, families, and multidisciplinary staff.
Availability to start on or around the stated start date and complete the 13‑week contract.
