HOME HEALTH RN | SUMMARY AND HIGHLIGHTS: The Home Health RN will implement care plans, supervise staff and LPNs, and coordinate with multidisciplinary health providers to ensure seamless transitions of care and regulatory compliance. Additionally, the role involves conducting health and safety training for new employees and unlicensed staff to ensure person-specific care standards are met. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH RN | RESPONSIBILITIES: Clinical Oversight: Observe, assess, plan, and evaluate nursing support for well, chronic, or acutely ill individuals. Case Management: Coordinate communications among various healthcare disciplines and ensure health and safety supports are integrated. Medication Management: Order, review, monitor, and dispose of medications as prescribed; ensure strict adherence to administration and storage policies. Supervision & Training: Direct the activities of LPN/LVN staff and provide health-related training and recertification for unlicensed personnel. Documentation: Complete daily, monthly, and annual nursing reports; accurately transcribe physician orders and maintain detailed progress notes. Coordination: Act as a liaison with inpatient nursing and discharge coordinators during hospitalizations to ensure seamless transitions of care. HOME HEALTH RN | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. License: Active and valid Registered Nurse (RN) license. Previous experience in case management or home health settings is highly preferred. Technical Skills: Familiarity with HCHB and OASIS documentation is a plus. Ability to cover assignments within Mesa and Garfield counties.
