Social Worker - Medical Case Manager Part Time Days

Position Summary
The Social Work Case Manager is an integral member of the multidisciplinary care team, responsible for planning, organizing, and implementing social work services for patients and families based on individualized needs. This role ensures that services align with maintaining a patient‑focused approach to promote health restoration and achievement of patient goals.

Job Duties & Essential Functions

Conduct comprehensive biopsychosocial assessments to identify patient needs, barriers to care, and available supports.

Develop, implement, and monitor individualized care plans in collaboration with patients, families, physicians, and interdisciplinary healthcare teams.

Coordinate medical, behavioral health, and social services to ensure continuity and quality of care.

Assist patients with discharge planning, transitions of care, and follow‑up services.

Provide counseling, crisis intervention, and emotional support to patients and families coping with illness, injury, or chronic conditions.

Advocate for patients’ rights, access to services, and appropriate levels of care.

Connect patients with community resources such as housing, financial assistance, transportation, home health services, and support groups.

Educate patients and families on diagnoses, treatment plans, advance care planning, and available support services.

Maintain accurate and timely documentation in electronic health records in compliance with regulatory and organizational standards.

Collaborate with insurance providers and utilization review teams to support authorization and coverage of services.

Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.

Ensure compliance with ethical standards, confidentiality requirements, and applicable state and federal regulations.

Ability to advocate for patient needs and rights throughout the care continuum.

Strong teamwork skills to work effectively with physicians, nurses, and other healthcare professionals.

Ability to assess complex psychosocial situations and develop appropriate interventions.

Sensitivity to diverse cultural, socioeconomic, and spiritual needs.

Adherence to NASW Code of Ethics and hospital policies.

Perform other related duties as assigned.

Required Experience & Qualifications

NY‑State Licensed Social Worker (LMSW or LCSW required).

Master’s degree in Social Work required.

Demonstrated experience in patient discharge planning and care coordination.

Familiarity with legal aspects of healthcare and community resources.

Excellent communication, problem‑solving, and interpersonal skills.

Preferred Experience & Qualifications

Experience in acute care or hospital setting preferred.

Knowledge of insurance regulations and utilization review processes preferred.

Physical Demands

Hospital‑based setting with frequent interaction with patients, families, and healthcare staff.

Ability to sit, stand, and walk for extended periods, occasional lifting of patient charts or supplies.

Stony Brook Medicine is a smoke‑free environment. Smoking is strictly prohibited anywhere on campus, including parking lots and outdoor areas on the premises.

All hospital positions may be subject to changes in pass days and shifts as necessary.

This position may require the wearing of respiratory protection, which may prohibit the wearing of facial hair.

This function/position may be designated as “essential.” This means that when the hospital is faced with an institutional emergency, employees in such positions may be required to remain at their work location or to report to work to protect, recover, and continue operations at Stony Brook Medicine, Stony Brook Eastern Long Island and related facilities.

Anticipated Pay Range
The starting salary range (or hiring range) for this position has been established based on relevant experience. The above salary range (or hiring range) represents SBEILH’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

Prior to Start Date

Successfully complete pre‑employment physical examination and obtain medical clearance from Stony Brook Eastern Long Island’s Employee Health Services *

Complete electronic reference check with a minimum of three (3) professional references.

Successfully complete a 4‑panel drug screen*

Meet regulatory requirements for pre‑employment screenings.

Provide a copy of any required New York State licence(s)/certificate(s).
