Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness in Arizona.

Case Manager
The Case Manager provides case management services in the community and/or office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching and advocating, coordinating and monitoring the patient’s needs and progress. They help patients gain access to health services, financial assistance, housing, employment, education, social services and natural supports; assess patient needs, develop and monitor individualized treatment plans, and document services accurately and promptly. The Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The role also involves implementing advanced techniques and interventions for high‑risk individuals in the associated program.

Skills/Requirements

High school diploma or GED required. Associate’s Degree or higher in a field related to behavioral health preferred.

1‑3 years of experience working with the GMH/SU population required.

1‑3 years of experience in a behavioral health position providing outreach and engagement activities required.

1‑3 years of experience in a medical setting preferred.

Minimum of six months of recovery from substance use and/or mental health disorders preferred.

Or equivalent combination of education and experience.

Current AZ Driver’s License (valid and in good standing preferred).

Valid ID required.

Clean 39‑month driving record (MVR) preferred.

Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures.

Arizona Fingerprint Clearance card.
