Overview
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Position
Client Care Coordinator
The Community Bridges, Inc. (CBI) Client Care Coordinator is a shelter or temporary lodging site-based position with a primary role of managing a housing caseload and coordinating care for individuals and families who are experiencing homelessness. The position provides coordination of care and support services to assist the individual or family with ending their current episode of homelessness. These individuals may present with complex substance use, mental health, housing, and medical care needs. The role ensures coordination of care with existing PCMH/ PCP or connects to new PCMH/ PCP services. The Client Care Coordinator works with individuals and families with the highest acuities and greatest vulnerabilities. The role works as part of a multi-disciplinary team including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads.
Responsibilities
Coordinate care for individuals and families experiencing homelessness and manage a housing caseload.
Coordinate with existing PCMH/PCP or connect to new PCMH/PCP services.
Collaborate with a multi-disciplinary team to support clients’ behavioral and medical needs.
Qualifications
High school diploma or GED required. Associate degree (or higher) in a field related to Behavioral Health preferred.
Minimum of six months of recovery from substance use and/or mental health disorders required.
Minimum of six (6) months of paid related work experience providing exposure to behavioral and medical crisis situations required.
1-3 years of full-time health care related work experience with exposure to behavioral and medical crisis situations preferred.
