St. Luke's Health System, Meridian, ID, United States

Overview
At St. Luke’s, our Pharmacy Technicians play a vital role in delivering outstanding, patient-focused care. We are committed to creating an environment that values and leverages the unique strengths, experiences, and perspectives of our employees, which ultimately enhances our exceptional, patient-centered care.

Medication Historian Technicians (MHTs)

play a vital role in ensuring accurate and complete medication histories, directly supporting safe, high-quality patient care.

Responsibilities

Patient-facing care (onsite and virtual): MHTs interact directly with patients either at the bedside or through virtual care technology to review and clarify current medication use. This role offers meaningful, direct patient interaction every day.

Pharmacy and provider outreach: Technicians verify recently filled medications by contacting community pharmacies and, when needed, reach out to patients’ primary care and specialty providers to clarify medication regimens and ensure the most accurate medication list possible.

Medication list review: MHTs independently identify discrepancies and gaps in therapy, which are reviewed and validated by pharmacists to ensure safe and accurate medication information.

Interprofessional collaboration: MHTs work closely with providers, inpatient pharmacists, nurses, and case management/social work teams to communicate key findings and support continuity of care.

Independent, specialty role: This is a highly autonomous position where technicians manage their own workload and practice at the top of their license. For those seeking a specialized role with ownership, accountability, and clinical impact, Medication History is an excellent fit.
