What You Will Do
Follow the individualized service plan (ISP) for each assigned resident
Observe residents for changes in condition and report any concerns immediately to the nursing supervisor
Document daily care, including observations and any changes throughout the shift, especially during rounds
Check and record vital signs, weights, and perform other duties in accordance with state regulations and community policies
Assist residents with activities of daily living (ADLs), including bathing, grooming, oral care, toileting, and dressing
Perform restorative care techniques such as proper positioning, ADL support, and passive range of motion exercises
Provide redirection, cues, and supportive communication to residents as needed
Encourage and assist residents in participating in scheduled activities
Assist with transfers and ambulation using proper body mechanics and equipment
Provide transportation within the community and accompany residents to appointments as needed
Respond promptly to call systems and resident needs
Perform other duties as assigned by the supervisor
Why You Are Qualified
High school diploma or equivalent (GED) required
Nursing Assistant Training Program certification (as required by state regulations)
CPR and First Aid certification (if required)
Ability to perform physical tasks including standing, walking, bending, lifting, and assisting residents throughout the shift
Must be able to lift up to 50 pounds and assist with resident transfers using proper equipment
