What You Will Do
Provide direct resident care services, including medication administration and assistance
Follow individualized service plans (ISP) for all residents
Perform resident assessments in accordance with state regulations and community policies under the direction of the Director of Assisted Living
Observe residents for changes in condition and report concerns promptly to the supervisor and/or Executive Director
Provide timely interventions for changes in condition and communicate updates to care staff and families as appropriate
Accurately document all observations, incidents, interventions, and medication assistance throughout the shift
Communicate effectively with physicians, families, and team members as needed to support quality care
Ensure proper shift-to-shift communication to maintain continuity of care
Support and oversee timely documentation by Resident Assistants, including services provided and any noted changes
Maintain and update individualized care plans for each resident
Perform other duties as assigned by the supervisor
Why You Are Qualified
High school diploma or equivalent (GED) required
Minimum of one (1) year of related experience preferred
Medication Technician certification (as required by state regulations)
Strong attention to detail and ability to follow medication administration protocols
Ability to perform physical tasks including standing, walking, bending, and assisting residents throughout the shift
Must be able to lift up to 25 pounds unassisted
