
Case Manager (ASO2) - Veterans Treatment Court

Overview
The Case Manager (ASO2) is a non-Civil Service position located within the Veterans Treatment Court of General Sessions Court. This position provides comprehensive case management services to justice-involved veterans who have been diagnosed with a mental health disorder, substance use disorder, or who have co-occurring disorders. They shall perform other duties as assigned.
Responsibilities
Works closely with staff from the Veterans Administration, specifically the VJO and Peer Support Specialist.
Collaborates with the Veterans Mentor Coordinator and their volunteers.
Makes referrals to and collaborates with Veterans Service Officers.
Performs intake interviews with applicants to the program.
Determines needs and formulates a case plan to address both criminogenic and non-criminogenic needs as they present.
Works with clients to address their court requirements and case plan through in-person meetings.
Makes referrals to community mental health agencies, substance abuse treatment agencies, and other community resources.
Works in collaboration with community resources to achieve treatment goals identified for the client.
Provides weekly reports, both verbal and written, to the Treatment Team regarding client progress.
Collaborates and communicates effectively with the multi-disciplinary treatment team.
Recommends therapeutic adjustments, sanctions, and incentives to the Treatment Team.
Manages client files, both electronic and written, including data entry into both state and local databases.
Conducts visits with clients in the community and at their home.
Effectively communicates with clients and their support system.
Liaisons between client and their families, community resources, and other pertinent entities.
Maintains knowledge of best practices in supervision and treatment of clients diagnosed with a mental illness and/or a substance use disorder.
Engages in ongoing training (both self-led and formal) to better understand the All Rise Best Practice Standards, HIPAA, CFR, mental health disorders and treatment, substance use disorders and treatment, trends in substance misuse, etc. This includes the mandatory attendance of both state and national trainings as directed.
Education and Experience
Any combination of education and experience that would prepare the incumbent to perform duties of the position at the appropriate level. Employees would typically have a Bachelor\'s Degree plus some (1-2 Years) professional experience in an administrative, financial, technical, or related field.
Note: Candidates with accreditations earned in a foreign institute are encouraged to apply.
Note: Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.
Experience working in Recovery Courts/Problem Solving Courts/Treatment Courts
Experience working with adults experiencing mental health and substance use disorders
Knowledge of community resources to residents of Davidson County
