Overview
Physical Therapist opportunity in Milford, New Hampshire. This travel, temporary assignment starts 06/08/2026 and runs for 13 weeks in a skilled nursing setting. Responsibilities
Conduct initial evaluations and ongoing assessments of residents to determine PT needs; develop and implement individualized treatment plans.
Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, gait, and functional independence.
Reassess progress regularly, adjust plans of care, and document outcomes in compliance with regulatory and facility standards.
Collaborate with an interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate resident care.
Educate residents and families on home exercise programs, safety strategies, and activity pacing to support continued improvement.
Maintain accurate documentation and uphold safety, privacy, and infection control guidelines for all encounters.
Requirements
Active Physical Therapist license for New Hampshire or eligibility to obtain licensure.
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
Preferred: minimum 1 year of PT experience with geriatric populations or in SNF/long-term care settings.
Ability to work on-site in Milford, NH for a 13-week travel assignment; reliable transportation and schedule flexibility as needed.
Strong communication, documentation, and clinical reasoning skills; professional demeanor and teamwork orientation.
