Per Diem MSW Case Manager — Hospital Discharge & Counseling

Atlantic Health System is hiring a per diem Social Worker (MSW) for Case Management at Newton Medical Center. Responsibilities include discharge planning and providing supportive and therapeutic counseling. Candidates must have a minimum of an MSW and an Active LSW from NJ, along with at least one year of medical experience.
