Client Care Coordinator
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency that provides a range of services across Arizona. The Client Care Coordinator works at a shelter or temporary lodging site during the peak heat season (May–October) and may continue in related roles within the agency if available.
Key Responsibilities
Manage a housing caseload by coordinating care and support services for individuals and families experiencing homelessness.
Provide case management and coordination with primary and specialty care providers to address medical, behavioral, and housing needs.
Use housing search software and other tools to identify and match clients to appropriate housing units.
Document and track required interactions in accordance with Arizona Administrative Code (AAC) and CBI policies.
Complete daily data entry and generate end‑of‑shift reports on care coordination activities.
Collaborate with a multidisciplinary team—including nurses, counselors, and clinical leads—to ensure seamless service delivery.
Respond to urgent heat‑related health issues and connect clients with appropriate resources.
Required Qualifications
High school diploma or GED; an associate degree or higher in behavioral health preferred.
Minimum 12 months of recovery from substance use and/or mental health disorders.
Minimum six (6) months of paid related work experience with exposure to behavioral and medical crisis situations.
1–3 years of full‑time health‑care related work experience preferred.
Valid identification.
Will complete CBI Peer Certification within 90 days of hire.
Arizona Fingerprint Clearance Card (program specific).
