Job Summary
The Client Care Coordinator is a shelter or temporary lodging site‑based position responsible for managing a housing caseload and coordinating care for individuals and families experiencing homelessness. The role functions seasonally from May to October, with potential for continued employment if suitable positions become available.
Key focus
is delivering community‑based services to help clients transition from homelessness to stable housing.
Responsibilities
Coordinate and monitor care for individuals residing in shelters or temporary lodging, ensuring timely delivery of RBHA and non‑RBHA community services.
Work with high‑acuity clients, liaising with PCMH, PCP and other providers to connect clients with medical, behavioral, and medication‑assisted treatment.
Facilitate housing searches using the agency’s software, assist clients in identifying suitable units, and coordinate move‑in processes.
Maintain accurate data entry in the agency’s systems and complete end‑of‑shift reports documenting care coordination for each client.
Document and track required interactions in compliance with the Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, and CBI policies.
Collaborate with a multidisciplinary team—including nurses, counselors, and clinical leads—to ensure integrated care.
Engage community partners and local service providers to distribute resources and address urgent needs, especially during extreme heat events.
Qualifications
High school diploma or GED required; associate degree or higher in a behavioral health related field preferred.
Minimum six months of recovery from substance use and/or mental health disorders.
Minimum six months of paid relevant work experience with exposure to behavioral and medical crisis situations.
Preferred 1–3 years full‑time health‑care related experience.
Valid identification; Arizona Fingerprint Clearance Card.
Will complete CBI Peer Certification within 90 days of hire.
