Monitor progress, document outcomes, and adjust treatment plans as needed.

Collaborate with an interdisciplinary team to support comprehensive resident care and discharge planning.

Reassess patient progress regularly and communicate changes in goals and strategies to the care team.

Requirements

Active physical therapy license in Maryland, or eligibility to practice in Maryland.

Eligible for travel assignments and able to work in a long‑term care environment.

Strong written and verbal communication skills; proficient in documentation.

Availability to work the stated shift (8:00 AM–4:30 PM), five days per week.

Demonstrated ability to function effectively in a collaborative, interdisciplinary setting.
