What You’ll Do
Provide individualized speech, language, and swallowing therapy in clients’ homes
Conduct diagnostic assessments and create tailored treatment plans
Offer functional communication and cognitive-linguistic training
Educate clients and caregivers on strategies that reinforce progress and safety
Document services using EMR and communicate with the care team as needed
Qualifications
Current
Minnesota Speech Language Pathologist (SLP)
license
Valid driver’s license and reliable transportation
Experience with EMR preferred
Passion for patient-centered, one-on-one home care
